By sending my request for affiliation, I certify that:
- I join the Securex Integrity Social Insurance Fund
- The data for this connection are genuine and complete
- That I will report all changes to information applicable to my connection to Securex within fifteen days
- That I was clearly, correctly and adequately informed about and aware of :
- The rules applicable for determining the nature of the labor relations law
- The fact that inaccurate and false statements are punishable offenses
- The existence of operating costs, the calculation of operating costs and the services to which they are entitled, in accordance with article 20,§4 of the Royal Decree of July 27, 1967
- The provisional contributions and their regularisation in accordance with article 41 bis of the Royal Decree of December 19, 1967
- That any natural person exercising a self-employed professional activity in Belgium must be affiliated at the latest on the day of the start of the self-employed activity
- That each self-employed person must register in advance in the CBE for all his self-employed activities
- That in case of non-compliance with these obligations, an administrative fine will be imposed by the competent governmental authorities
- That legal entities are jointly and severally liable for the payment of the fine imposed on their partners or mandataries
- That it is a legal obligation to report changes to the information applicable to the affiliation to the social insurance fund within fifteen days
